Erythrocyte sedimentation rate’s principle

Erythrocyte sedimentation rate’s principle

Erythrocyte sedimentation rate is the rate at which erythrocytes naturally sink in in vitro anticoagulated whole blood under specified conditions.

Erythrocyte sedimentation rate's principle

The saliva on the surface of the red blood cell membrane in the bloodstream repels each other due to the negative charge and other factors, so that the distance between cells is about 25nm, the protein content is higher than that of plasma, and the specific gravity is greater than that of plasma. So they disperse and suspend each other and sink slowly. If the plasma or red blood cells themselves are altered, the erythrocyte sedimentation rate can be altered.

There are three stages of erythrocyte subsidence

① erythrocyte coin-shaped aggregation stage: the "disk-shaped planes" of erythrocytes adhere to each other to form erythrocyte coin-shaped strings. On the basis, for each additional red blood cell that fits, two more "disc planes" are eliminated. This process takes about 10 minutes;

② Rapid erythrocyte sedimentation period: the number of erythrocytes that adhere to each other gradually increases, and the sinking speed is accelerated, and this stage lasts about 40 minutes;

③ erythrocyte accumulation period: the number of erythrocytes that adhere to each other reaches saturation and decreases slowly, and the close Stack with bottom of container. The reason for the manual Wilcoxon method requiring reporting of ESR results at the end of 1 hour.

Erythrocyte sedimentation rate's determination

There are many methods, including Wei's method, Ku's method, Wen's method and Pan's method. The difference lies in the anticoagulant, blood volume, erythrocyte sedimentation rate tube, observation time and recording results. The Kurt's method records the results every 5 minutes. In addition to obtaining the sedimentation results of 1 hour, it can also see the sedimentation curve during this period, which has a certain value in the judgment of the activity of tuberculosis lesions and prognosis. The correction curve of erythrocyte sedimentation rate in anemia is proposed, or the influence of anemia on erythrocyte sedimentation rate results is eliminated. Pan's method does not need to collect blood from veins, but only needs blood from the fingertips, but it is often affected by the mixing of tissue fluids. Each of the above methods has its advantages and disadvantages.
