1 Indications
Puncture closed drainage is mainly applicable to tension pneumothorax or pleural effusion.
2 Puncture procedure
1. For those who cough more frequently, 0.03 ~ 0.06g codeine should be taken orally before operation to avoid sudden severe cough during operation, affecting operation or needle tip stabbing lung.
2. The puncture site is the same as the entrance of closed thoracic drainage.
3. The skin was routinely disinfected, sterile surgical towels were paved, and routine local anesthesia was performed until the pleural layer.
4. Make a small incision of 0.5cm with a sharp knife into the skin at the point of needle entry until subcutaneous; The trocar was inserted slowly from the skin incision to the chest; Pull out the needle core, quickly insert the porous silica gel tube at the front end, and exit the sleeve; The silica gel tube is connected with the water sealed bottle; Sew a needle with medium-sized silk thread at the pinhole, and fix the drainage tube on the chest wall. If it is necessary to record the air extraction volume, connect the drainage tube to the artificial pneumothorax device, record the air extraction volume and observe the change of thoracic pressure.
3 Precautions
1. The entire operation should be strictly aseptic procedures to prevent secondary infection, and the puncture and drainage place should be covered with sterile gauze.
2. Pay attention to strictly implement the requirements of "double fixation" of the drainage tube, and fix the rubber tube connecting the water sealing bottle on the bed surface with adhesive tape.
3. Other precautions are the same as closed thoracic drainage.
