Laparoscopic trainer improves surgery skills
Use a simple laparoscopic trainer for basic operation training under the microscope
This teaching experiment is mainly aimed at two groups of refresher doctors who participated in the improvement class of attending doctors in Shaanxi Province in our department from 2013 to 2014. All doctors are attending doctors of clinical general surgery in secondary hospitals with certain working experience, and all have certain experience in laparoscopic surgery. A total of 32 people, 16 of whom (designated as group A) received 2-hour laparoscopic trainer operation training every day for 2 months in addition to daily clinical work. The other 16 (group B) directly followed the accompanying teachers to carry out various operations every day, including laparoscopic surgery. The trainer used this time is a simple laparoscopic trainer, including chassis, retractable and directional camera, display and laparoscopic instruments.
Various templates can be placed in the trainer box to complete the following basic operation training:
(1) picking up soybeans under the mirror: a handful of soybeans and a narrow mouth bottle are placed on the bottom plate of the training box, and the soybeans are moved into the narrow mouth bottle one by one with left and right hand-held grasping pliers to train accurate positioning and orientation skills.
(2) Artificial blood vessel ligation: fix the artificial plastic tube on the bottom plate, hold the thread with both hands, pass the thread and tie the knot, and train the movement coordination of holding the weapon with both hands.
(3) Suturing under microscope: artificial skin incision is placed on the bottom plate and sutured and knotted under the microscope, which is suitable for the most basic suturing operation under the microscope. The three kinds of basic operation training are progressive exercises. The second step of artificial vessel ligation training can be carried out only when both hands pick up soybeans alternately for 20 / min. Suture training can only be carried out after knotting for 5 times / min under the microscope. The suture requires 3 stitches, knotting and thread cutting to be completed within 10 minutes. After daily uninterrupted training, trainees can meet the above requirements within one month.
Finally, those who pass the examination shall be arranged to operate the experimental animal (rabbit). After anesthesia, the abdominal wall of the rabbit shall be cut and fixed on the test bench:
(1) Expose the intestinal tube, cut the intestinal tube under conventional microscope and suture the intestinal tube continuously.
(2) Cut the renal capsule and lateral peritoneum, double ligate and cut off the renal artery and vein, and complete the nephrectomy. Through the above exercises, the purpose of training the operation skills such as anatomy, separation, cutting, knotting and suture under endoscope can be achieved.
